Primary care providers are increasingly becoming responsible for full risk of their patients nationwide. To enable this responsibility, practices are building value aligned networks to manage their census in the community. Concurrently, the need for home care providers to realize a fairer share of the premium dollar means they are destined for closer proximity to the world of primary care. Some will integrate fully and become a piece of a shared parent entity to enable capitation. Others will coordinate closely - and share risk - with medical practices. Either way, the days of being purely a downstream specialist in traditional home health and hospice FFS are dwindling. Our panel will debate the merit of a full menu of approaches for making this leap from in-home care specialist to longitudinal, complex care partner.
