What is your palliative care strategy and how is it working for you - financially and operationally?

What are you doing outside of palliative care to broaden your capabilities and appeal as cost-saving entity in the serious illness continuum?

How are you participating in ACO REACH now, and what is your approach to REACH and other risk-bearing models?
Following the roundtable, we will move ahead with an ongoing workgroup that will have an established curriculum for each monthly/semi-monthly meeting.
