September 24 - Workgroup: Serious Illness

This is the launch meeting for a rotating serious illness workgroup, to be hosted by our Executive Advisor Hank Ross. The main focus is the upstream move by traditional hospice and palliative care providers to enable patient-centered care while preventing unnecessary care and cost, thereby meeting a critical gap in the drive for accountable care. Initial workgroup curriculum will focus on:

1. 1. Creating financially viable palliative/supportive care practices.

   2. Aligning with and/or building/buying home-based primary care practices.

   3. Participating in ACO REACH and other accountable/value-based care models.
